Prolotherapy

Prolotherapy is an injection-based approach used for selected joint, ligament, and tendon concerns. At OWM Integrative Wellness, we use a concentrated dextrose solution within individualized treatment plans intended to improve comfort and physical function.

Sometimes called ligament and tendon reconstruction therapy or fibro-osseous injection therapy, prolotherapy is considered after an assessment of your symptoms, joint stability, movement, and previous treatment. These names describe the approach rather than guaranteeing structural reconstruction.

How Prolotherapy Works

Targeted Dextrose Injections
Concentrated dextrose is injected into selected ligament attachments, tendon attachments, joint capsules, or joint spaces. The treatment is intended to produce a local response that may influence tissue repair signaling and pain.

The Proposed Repair Response
Research explores how dextrose affects inflammatory signaling, connective tissue activity, and collagen production. Some studies report improvements in pain and function, although the mechanisms remain incompletely understood and results vary by condition.

Clinical improvement does not necessarily establish that an injection recruits stem cells, restores cartilage, or permanently stabilizes a joint.

The OWM Approach

Hackett-Hemwall Principles
We assess the supporting structures around a joint as well as the joint itself. Hackett-Hemwall principles inform a comprehensive approach that may include relevant ligaments, tendon attachments, and the surrounding capsule.

Treatment sites are selected according to clinical findings, with attention to how these structures contribute to movement and load distribution.

Ultrasound Guidance
We use real-time, high-resolution ultrasound to visualize the selected anatomy and guide needle placement. Imaging supports precision and helps identify nearby structures, while careful technique and appropriate patient selection remain essential.

Treatment Variations

Concentrated Dextrose Prolotherapy
This approach may be considered for selected ligament-related symptoms, tendon disorders, and osteoarthritis. Suitability depends on the diagnosis, severity, and response to other care.

Prolozone Therapy
Our Prolozone protocols combine prolotherapy principles with medical ozone and selected injection solutions. We explain the preparation, intended role, evidence, and risks separately from dextrose prolotherapy.

PRP-Based Treatment
Platelet-rich plasma uses concentrated platelets from your own blood and may be incorporated into a broader injection plan. PRP and dextrose are distinct preparations, with their roles assessed individually for arthritis, tendon symptoms, or selected structural injuries.

Neural Prolotherapy & Hydrodissection
Dilute dextrose may be used around selected peripheral nerves. When fluid is introduced specifically to separate a nerve from surrounding tissues, the procedure is called hydrodissection.

This differs from concentrated dextrose treatment of ligaments and tendons. Evidence for one application does not establish effectiveness for another.

Supporting Treatments

Peptide Preparations
Selected OWM protocols include BPC-157 and thymosin beta-4 within proposed injection solutions at no additional charge. CJC-1295 or ipamorelin may also be discussed before certain procedures.

We review these preparations separately, including their evidence, regulatory status, and potential risks. Their use should not be presented as an established method of stimulating stem cell activity or enhancing prolotherapy outcomes.

Interstitial Low-Level Laser Therapy
Selected plans may incorporate Weber Endo Laser fiberoptic light delivery within the treatment area, using wavelengths such as red, blue, green, or yellow.

We explain the proposed application before treatment. Improved growth-factor activity, cellular energy production, or healing from the combined protocol has not been established.

Conditions We Evaluate

Spine & Neck
Assessment may include chronic neck, mid-back, lower-back, or sacroiliac pain, as well as facet-related symptoms and degenerative spine concerns. We first determine which structures are likely contributing to discomfort.

Joint Pain & Osteoarthritis
Areas evaluated include the knees, hips, shoulders, elbows, wrists, hands, ankles, and feet. Evidence and suitability vary across joints and stages of arthritis.

Tendon & Ligament Concerns
Selected conditions may include rotator cuff symptoms, tennis or golfer’s elbow, Achilles tendinopathy, ACL or MCL sprains, and plantar fascia pain. Significant tears or instability may require rehabilitation or surgical assessment.

Temporomandibular Joint Concerns
We may evaluate jaw pain, clicking, or suspected hypermobility to determine whether an injection-based approach has an appropriate role alongside dental or other specialist care.

Treatment & Follow-Up

Before treatment, we explain the selected preparation, injection sites, expected recovery, and alternatives. Temporary soreness, swelling, or stiffness may occur afterward.

The number and spacing of sessions depend on your diagnosis and response. Follow-up focuses on pain, movement, and daily function, with rehabilitation and activity guidance incorporated when appropriate.

Potential risks include bruising, bleeding, infection, reactions to ingredients, and injury to nearby structures. Dextrose’s familiarity as a substance does not make an injection risk-free.
